ROUNDY’S SUMMER HEALTHY FUN FOR THE WHOLE HERD SWEEPSTAKES OFFICIAL RULES - NO PURCHASE NECESSARY.
A PURCHASE DOES NOT INCREASE CHANCES OF WINNING.
VOID OUTSIDE OF WI, MN, IA, IL, MI AND WHERE PROHIBITED.

1. HERE’S HOW TO ENTER

There are three ways to enter the Roundy’s Summer Healthy Fun for the Whole Herd Sweepstakes (“Sweepstakes”), beginning 12:00:01 AM ET June 1, 2009 and ending 11:59:59 PM ET June 30, 2009:

 

(a). To enter, purchase any three (3) participating products (refer to Addendum “A” for a complete list of products) in a single transaction at a participating Roundy’s Supermarket location during regular store hours from June 1, 2009 through July 30, 2009. Then, visit www.healthyfortheherd.com to download and print the UPC entry form. Complete the UPC entry form by providing all requested information and writing down 3 UPC codes from the list of participating products (that are included on the receipt from your transaction). Entrants must mail their UPC entry form in a stamped #10 envelope to: Roundy’s Summer Healthy Fun for the Whole Herd Sweepstakes, P.O. Box 551590, Jacksonville, FL 32255-1100. Mail-in entries are limited to 1 entry per stamped envelope. Each mail-in entry must be postmarked by June 30, 2009 and received by July 7, 2009. Neither Roundy’s Supermarkets, Inc. (the “Sponsor”), Promo Depot MatchPoint, LLC, their respective parent companies, subsidiaries, affiliates and the directors, officers, employees or agencies, nor any of their respective partners or prize suppliers (collectively, the “Sponsor Affiliates”) are responsible for lost, late, damaged, delayed, destroyed, misdirected, incomplete, illegible, mutilated or postage due mail. Photocopies, mechanically reproduced, forged or altered mailed entries and facsimiles are void. Incomplete entries are not eligible. Mail-in envelopes larger than a #10 envelope will be automatically disqualified.

 

(b.) To enter online, entrants may visit www.healthyfortheherd.com and complete an online entry form, including all requested information. Online entries must be received by 11:59:59 PM ET June 30, 2009 and must be complete in order to be eligible. Online entries generated by script, macro or other automated means are void. Normal Internet access and usage charges imposed by entrants’ online service will apply and shall be the sole responsibility of the entrant. Neither the Sponsor nor Sponsor Affiliates assume any responsibility for incorrect or inaccurate capture of entry information, including, but not limited to, technical malfunctions, human or technical error, printing errors, lost/delayed/garbled data or transmissions, omission, interruption, deletion, defect or failures of any telephone or computer line or network, computer equipment, software or any combination thereof.

 

(c). On a plain 3 1/2” x 5” piece of paper, entrants may hand print their complete name, address, city, state, zip code, daytime phone number, email address and date of birth. Entrants must mail their entries in a stamped #10 envelope to: Roundy’s Summer Healthy Fun for the Whole Herd Sweepstakes, P.O. Box 551590, Jacksonville, FL 32255-1100. Mail-in entries are limited to 1 entry per stamped envelope. Each mail-in entry must be postmarked by June 30, 2009 and received by July 7, 2009. Neither the Sponsor or Sponsor Affiliates are responsible for lost, late, damaged, delayed, destroyed, misdirected, incomplete, illegible, mutilated or postage due mail. Limit two entries, per person/e-mail address, regardless of method of entry, per day.

 

2. ELIGIBILITY

The Sweepstakes is only open to legal U.S. residents residing in Wisconsin, Minnesota, Iowa, Illinois or Michigan, who are 25 years of age or older as of June 1, 2009. Employees of the Sponsor and Sponsor Affiliates and any of the members of their immediate family (defined as spouse, mother, father, in-laws, grandmother, grandfather, brother, sister, children and grandchildren) or same household of each are not eligible to participate. This Sweepstakes is void outside of Wisconsin, Minnesota, Iowa, Illinois, Michigan and where prohibited or restricted by law, and is subject to all applicable federal, state and local laws.

 

3. GRAND PRIZES AND APPROXIMATE RETAIL VALUE (“ARV”)

Two (2) Grand Prizes – A four (4) day, three (3) night trip for four (4) to Los Angeles, CA. Trip includes roundtrip coach class air transportation for four (4) from major airport in Minneapolis, Milwaukee, Chicago, Detroit or Des Moines to Los Angeles, CA; three (3) nights hotel accommodations at a Sponsor specified hotel, based on quadruple occupancy; use of car rental; four (4) tickets to Disneyland Park; four (4) tickets to Universal Studios; four (4) tickets to a Los Angeles Dodgers baseball game at Dodger stadium in Los Angeles, CA on date specified by Sponsor; a pre-paid $500 Sponsor specified gift card which winner may use for admission for four (4) into the La Brea Tar Pits in Los Angeles, CA or if winner does not wish to visit the La Brea Tar Pits, he/she may use gift card for meals and/or shopping. Certain restrictions and an expiration date may apply for the gift card. ARV of each Grand Prize is up to $6,694 (actual ARV will depend on winner’s point of departure). Sponsor reserves the right to substitute prize (or portion of the prize) with one of equal or greater value if the prize cannot be awarded as described for any reason. Total ARV of all Grand Prizes is up to $13,388.

 

4. FIRST PRIZES AND ARV

Twenty-five (25) - A Trek Mountain Bike. ARV of each First Prize is $350. Sponsor reserves the right to substitute prize with one of equal or greater value if the prize cannot be awarded as described for any reason. Total ARV of all First Prizes is $8,750.

 

5. SECOND PRIZES AND ARV

Two hundred fifty (250) – A pair of movie tickets to see Ice Age 3: Dawn of the Dinosaurs. ARV of each Second Prize is $20. Movie tickets are subject to restrictions and an expiration date. Sponsor reserves the right to substitute prize with one of equal or greater value if the prize cannot be awarded as described for any reason. Total ARV of all Second Prizes is $5,000.

 

6. COUPONS AND ARV

Twenty thousand (20,000) – A coupon good for one of the four products listed below (in Sponsor’s sole discretion). The Sponsor reserves the right to substitute the coupon with coupon of comparable or greater value if the coupon cannot be awarded as described for any reason. Total ARV of all coupons is $66,800.

7. TOTAL APPROXIMATE RETAIL VALUE OF ALL PRIZES/COUPONS

$93,700.

 

8. RANDOM DRAWING

The winners will be selected in a random drawing on or about July 10, 2009, from among all eligible entries received. The drawing will be conducted by Promo Depot MatchPoint, LLC, whose decisions are final and binding on all matters related to this Sweepstakes. Odds of winning are dependent upon the number of eligible entries received for the drawing.

 

9. WINNER NOTIFICATION/VERIFICATION

The potential Grand Prize winners will be notified by phone and or mail and will be required to complete, sign and return, a notarized Affidavit of Eligibility/Release of Liability and, where legal, Publicity Release. The potential Grand Prize winners are required to return the document within 7 days from the postmark date on the prize notification letter or prize may be forfeited and an alternate winner selected. Grand Prize winners’ travel companions (or parent/legal guardian on behalf of travel companion if travel companion is a minor) must execute a Release of Liability prior to departure. If the prize notification is returned as undeliverable, such prize will be forfeited, and an alternate winner will be selected. The potential First Prize winners, Second Prize winners and Coupon recipients will be notified by phone and/or mail. If any prize/coupon or prize/coupon notification is returned as undeliverable, such prize/coupon will be forfeited, and an alternate winner will be selected. Winners are responsible for any and all federal, state and local taxes. Limit one prize/coupon per person/family/household.

 

10. GENERAL RULES

The Sponsor reserves the right at its sole discretion to disqualify any individual who tampers or attempts to tamper with the entry process or the operation of the Sweepstakes or violates the Official Rules. Entrants agree to be bound by these Official Rules and the decisions of the judges. All aspects of the prizes/coupons are nontransferable and no substitutions are allowed except at the Sponsor’s sole discretion. No cash equivalent or exchange of prizes/coupons is permitted (except at the Sponsor’s sole discretion). Airline tickets are for travel from major airport in Minneapolis, Milwaukee, Chicago, Detroit or Des Moines to Los Angeles, CA. Airline carrier’s regulations and conditions apply. Trip may not be combined with any other offer and travel may not qualify for frequent flyer miles. Grand Prize winners and respective travel companions must travel together on the same itinerary. Grand Prize winners and travel companions must possess valid travel documents (i.e., valid photo ID) in order to travel. All travel must be completed by September 30, 2009. Certain restrictions and blackout dates may apply. Seat selection and timing of trip subject to availability and confirmation of reservations. No refunds or credit for changes allowed. Car rental is subject to additional restrictions set forth by car rental contract. If a Grand Prize winner does not meet rental car requisites/requirements that portion of the prize will not be awarded. All other expenses and costs, not expressly listed above, including, but not limited to, taxes, tips, entertainment, transfers, and transportation to airport to and from Grand Prize winners’ home residence are the Grand Prize winners’ sole responsibility. The winners agree to indemnify and hold harmless and waive any and all claims of liability of any kind whatsoever against the Sponsor and/or the Sponsor Affiliates, including any accident, loss, injury or death occurring or in any way related to the prizes/coupons or use/misuse thereof or while traveling to and/or from a location to obtain or use a prize/coupon. Neither the Sponsor nor the Sponsor Affiliates make any warranty, representation or guarantee, express or implied, in fact or in law, relative to the use of the prizes/coupons including, without limitation, quality, merchantability and fitness for a particular purpose. The winners, in accepting their prizes/coupons, consent to the use of their names, addresses (city, state), photographs and likenesses for purposes of advertising, trade and promoting in any and all media on behalf of the Sponsor, without further compensation, except where prohibited by law.

 

11. GOVERNING LAW & VENUE

The Pennsylvania Courts (State & Federal) shall have sole jurisdiction of any controversies regarding the promotion and the laws of the state of Pennsylvania shall govern the same. Each entrant waives any and all objections to jurisdiction and hereby irrevocably submits to the venue of those courts within the county of Allegheny, Pennsylvania.

 

12. WINNERS LIST/RULES

To receive a Winners List /or Rules, mail a self-addressed, stamped envelope, by September 30, 2009, to: Roundy’s Summer Healthy Fun for the Whole Herd Sweepstakes Winners List Request/Rules, PO Box 551595, Jacksonville, FL 32255-1219.

 

13. SPONSOR

The Sponsor of this Sweepstakes is Roundy’s Supermarkets, Inc., 875 East Wisconsin Avenue, Milwaukee, Wisconsin 53202.

 

14. ADMINISTRATOR

The Administrator is Promo Depot MatchPoint, LLC, Four Penn Center West, Suite 400, Pittsburgh, PA 15276.
